Aging, however undesirable, is a normal and expected part of the human condition. Yet, for those who study aging, there is a new take.
It starts with how the aging process works. As we age, our cells age – a process called cellular senescence – and the result is an accumulation of damaged cells. These cells then release inflammatory factors that act like signals to the immune system to clear the damaged cells – a process which works well when we are young. However, as we age, our cells aren’t cleared as effectively by our immune system, and as they begin to accumulate, they cause low level inflammation and release enzymes that can degrade tissue.
How to reduce the burden of damaged cells has been the question for University of Minnesota Medical School faculty Paul D. Robbins and Laura J. Niedernhofer and Mayo Clinic investigators James L. Kirkland and Tamara Tchkonia.
In a recent study it seems they may have found their answer. Using a natural product called Fisetin, which is found in many fruits and vegetables, on mice towards the end of life, the researchers saw improvements both in health and lifespan (Yousefzadeh et al., 2018).
Robbins and his team now dub Fisetin is a senotherapeutic that, they say, “extends health and lifespan” (Robbins 2018).
“These results suggest that we can extend the period of health, termed healthspan, even towards the end of life” (Robbins, 2018).
This study was also novel in that it used mass cytometry, or CyTOF, technology and applied it for the first time in aging research, which as Robbins notes, helped demonstrate not only that Fisetin works to counteract aging, but just how it works (Robbins, 2018).
What we can ascertain from work like this is something we might have already guessed – eating fruits and vegetables is good for us. What we now know, however, is that it might also help us live longer and healthier.

Nutrition and Mental Health: Advanced Clinical Concepts is a 1-hour online continuing education (CE/CEU) course that examines how what we eat influences how we feel, both physically and mentally. While the role of adequate nutrition in maintaining mental health has been established for some time, just how clinicians go about providing the right nutritional information to the patient at the right time – to not just ensure good mental health, but actually optimize mood – has not been so clear. With myriad diets, weight loss supplements and programs, clients often find themselves reaching for the next best nutritional solution, all the while, unsure how they will feel, or even what to eat to feel better. On the other side of the equation, clinicians so often face not just a client’s emotional, situational, and relational concerns, but concerns that are clearly mired in how the client feels physically, and what impact his/her nutritional health may have on these concerns. For example, research into the role of blood sugar levels has demonstrated a clear crossover with client impulse control. Additionally, the gut microbiome, and its role in serotonin production and regulation has consistently made clear that without good gut health, mitigating anxiety and depression becomes close to impossible.

Living a healthy lifestyle will likely help you live longer. But new research suggests living a healthier lifestyle could also increase executive function, which is the ability to exert self-control, set and meet goals, resist temptation, and solve problems.
If you stop and think about it, it makes sense. Resisting donuts and opting for kale, after all, takes a fair amount of self-control – as does getting up early to exercise.
Over time, suggests researchers, these behaviors reinforce one another in a sort of positive feedback loop.
Using data collected from 4,555 adults through the English Longitudinal Study of Aging, researchers analyzed the relationship between physical activity and executive function, adjusting for other variables such as age, gender, education, wealth and illness, and found evidence that the relationship between the two is bidirectional (Allan et al., 2016).
Specifically, individuals with poor executive function showed subsequent decreases in their rates of participation in physical activity and older adults who engaged in sports and other physical activities tended to retain high levels of executive function over time (Allen et al., 2016).
While this study focused on physical activity and its relationship to executive function, the researchers noted that a positive feedback loop between executive function and eating nutritious foods is quite plausible. Similarly, it is likely that negative feedback loops also exist, where unhealthy behaviors, such as smoking or drinking too much alcohol will be both a result of and a predictor of declining executive function (Allan et al., 2016).
This might help explain why executive function typically declines with age, as older people may become more likely to engage in unhealthy behaviors like remaining sedentary and less likely to maintain healthy but effortful behaviors like taking prescribed medication regularly.
The up side, however, is that the longer one can maintain high executive function, the longer and more easily that person can stave off behavior that will be detrimental to their health. Dr. Julia Allan explains, “People who make a change to their health behavior, like participating in physical activity, eating less processed food, or consuming more fruits and vegetables, can see an improvement in their brain function over time and increase their chances of remaining healthy as they age” (Allan, 2016).
With the world’s population of elderly folks to hit 1.5 billion by 2050, the connection between executive function – and specifically how it is mediated by and helps to mediate our health – could have major implications.

If you have not been trained in grief work, it can be intimidating to have a client expressing grief by crying, being upset, or angrily responding to you. You may not have easy access to a social worker, psychologist, or other mental health professional. The primary ideas to remember are these:
It is not about you. Grief is not a problem to be solved. It is a process to live through.
Talking to someone who is grieving stirs up your own feelings. This can trigger inappropriate responses to the grieving person. Think about what you are about to say, and why. If you start to talk about yourself, stop. This is not about you. It is about the grieving person.
If you are a problem-solver, you may be tempted to find a solution for the person’s grief. It may be appropriate to refer the person to a mental health professional, but you will not solve the problem and make the person feel better today, tomorrow, or even a month from now. Grief must take its course, and the course will be different for everyone. Your best response is to listen supportively, without judgment and without giving advice (Devine, 2017).
Use these basic ideas to respond to a grieving person:
Say, “I can’t imagine how you are feeling.” Do not tell the person you know how they feel. Their grief is unique to them. You may have a similar experience, but it will not be the same.
Ask, “What can I do to help right now?” The person may not know what you can do to help, but asking provides the person a small amount of control in a situation that feels wildly out of control.
Say “It sounds as if you loved her/him very much. Would you like to tell me what made him/her so special?”
Be silent with the person and wait. Offer a tissue if needed.
Say, “I am not sure what to say to comfort you, but please know I care” (What’s Your Grief, 2014).

Grief is the reaction to loss, and any kind of loss can trigger grief. People grieve for the loss of someone they love, but they also grieve for the loss of independence, usefulness, cognitive functioning, and physical abilities. Grief is also a lifelong process: a journey rather than a disease that is cured. It changes over time to deal with different kinds of losses. It is an experience that is intellectual, physical, spiritual, and emotional. It is affected by the person’s culture, support system, religious beliefs, and a host of other factors.

The Grieving Self is a 3-hour online continuing education (CE) course that looks at stories of the bereaved to determine the major issues to address to reconnect those who grieve to a stable sense of self. The annual number of deaths reported in the United States in the early part of this century was 2.4 million, about four per minute. This course looks at the stories of a few of those who are recently bereaved to determine the major issues for those who grieve: aloneness, loss of self, social connections, anniversaries and holidays, self and others’ expectations, the need to continue living, ambivalence of recovery, grief dreams, medical problems. Studies are reviewed which indicate some researchers’ conclusions as to: 1) Gender differences between men and women who grieve; there are important questions regarding the recruitment of subjects and the data gathering process for gender differences research. 2) And, who among the grief survivors are best served by counseling and psychotherapy. This author, while agreeing with much of the research, challenges the belief that the emotional loneliness suffered by the bereaved is the single, major dynamic of the bereaved, and can only be alleviated through passage of time. It is felt that an effort to reconnect those who grieve to a stable sense of self can help the bereaved regain better function and reduce the length of the time they are consigned to painfully distressing lives. Suicide Prevention: Evidence-Based Strategies is a 3-hour online continuing education (CE) course that reviews evidence-based research and offers strategies for screening, assessment, treatment, and prevention of suicide in both adolescents and adults. Suicide is one of the leading causes of death in the United States. In 2015, 44,193 people killed themselves. The Centers for Disease Control and Prevention (CDC) notes, “Suicide is a serious but preventable public health problem that can have lasting harmful effects on individuals, families, and communities.” People who attempt suicide but do not die face potentially serious injury or disability, depending on the method used in the attempt. Depression and other mental health issues follow the suicide attempt. Family, friends, and coworkers are negatively affected by suicide. Shock, anger, guilt, and depression arise in the wake of this violent event. Even the community as a whole is affected by the loss of a productive member of society, lost wages not spent at local businesses, and medical costs. The CDC estimates that suicides result in over 44 billion dollars in work loss and medical costs. Prevention is key: reducing risk factors and promoting resilience. This course will provide a review of evidence-based studies so that healthcare professionals are informed on this complex subject. Overcoming the Stigma of Mental Illness is a 2-hour online continuing education (CE/CEU) course that explores the stigmas around mental illness and provides effective strategies to overcome them. The Substance Abuse and Mental Health Services Administration (SAMHSA) defines mental illness stigma as “a range of negative attitudes, beliefs, and behaviors about mental and substance use disorders.” Mental health and substance use disorders are prevalent and among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. This course will explore the stigmas surrounding mental illness and provide effective strategies clinicians can use to create a therapeutic environment where clients can evaluate their attitudes, beliefs, and fears about mental illness, and ultimately find ways to overcome them. Grief: The Reaction to Loss is a new 2-hour online continuing education (CE/CEU) course that teaches healthcare professionals how to recognize and respond to grief.
Grief is the reaction to loss, and any kind of loss can trigger grief. People grieve for the loss of someone they love, but they also grieve for the loss of independence, usefulness, cognitive functioning, and physical abilities. Grief is also a lifelong process: a journey rather than a disease that is cured. It changes over time to deal with different kinds of losses. It is an experience that is intellectual, physical, spiritual, and emotional. It is affected by the person’s culture, support system, religious beliefs, and a host of other factors.
Grief is often not recognized by healthcare professionals, in patients or themselves. This course will teach healthcare professionals to recognize grief, as well as how to respond appropriately to the grieving person. The progression of aging and dying will be discussed in order to normalize the process, one of the most important aspects of working with a grieving person. Screening guidelines for depression, suicide risk, and grief are included, as are treatment strategies for anticipatory and complicated grief. They are supposed to be your golden years. Your work is done, your finances are in order, and now you can enjoy your life. However, for many people, reaching retirement age brings many new physical and psychological issues.
In a paper published in June in the journal Aging, scientists from the institute’s Medical Informatics and Systems Division found that spontaneous mutations occur in our bodies constantly, but the rate of change differed dramatically among various people.
These changes are often associated with diseases such as diabetes, kidney failure, cancer, rheumatoid arthritis, and Alzheimer’s disease, and are linked to exposure to various environmental stressors (Bavarva et al., 2014).
“We observed that certain portions of our genome age 100 times faster than others. Microsatellites, once considered ‘junk DNA,’ are known to be associated with many diseases. They change much faster than individual DNA bases (known as single nucleotide polymorphisms, or SNPs), so it is important that future studies look at this very dynamic part of the human genome,” explains Harold Garner, a professor of biological sciences and computer science at Virginia Tech and a professor of medicine at the Virginia Tech Carilion School of Medicine and Research Institute (Garner, 2014).
Things are not as simple as we once thought, and aging doesn’t seem to follow any sort of predictable pattern. Nor are the changes simply physical.
According to a longitudinal study of men and women ranging in age from 25 to 104, self-esteem rises steadily as people age but starts declining around the time of retirement.
Self-esteem, which is related to better health, less criminal behavior, lower levels of depression and, overall, greater success in life was found to be lowest among young adults but increased throughout adulthood, peaking at age 60, before it started to decline (Orth et al., 2016).
Further, on average, women had lower self-esteem than did men throughout most of adulthood, but self-esteem levels converged as men and women reached their 80s and 90s. Blacks and whites had similar self-esteem levels throughout young adulthood and middle age. In old age, average self-esteem among blacks dropped much more sharply than self-esteem among whites – even after controlling for differences in income and health (Orth et al., 2016).
Even more interesting was the finding that people of all ages in satisfying and supportive relationships tend to have higher self-esteem, however, despite maintaining higher self-esteem throughout their lives, people in happy relationships experienced the same drop in self-esteem during old age as people in unhappy relationships. Explains Kali H. Trzesniewski, PhD, of the University of Western Ontario, “Although they enter old age with higher self-esteem and continue to have higher self-esteem as they age, they decline in self-esteem to the same extent as people in unhappy relationships” (Trzesniewski, 2016).
While there are numerous theories as to why self-esteem peaks in middle age and then drops after retirement, such as a change in roles, an empty nest, retirement and obsolete work skills in addition to declining health, not one theory accurately explains the decline. However, through understanding the common physical and psychological challenges that the aging population faces, professionals who treat them can help ensure the highest levels of functioning, and a well-earned retirement.

It is well known that exercise, in general, helps combat the aging process. However, do we know which forms of exercise are most beneficial?
New research done by German center for Neurodegenerative Diseases, Magdeburg, Germany shows we may not.
Recruiting elderly volunteers with an average age of 68, researchers assigned them to an eighteen-month weekly course of either learning dance routines, or endurance and flexibility training. After assessing participants at the end of the study, Dr. Kathrin Rehfeld, the lead author and her colleagues found what we might expect: both forms of exercise produced increases in the area of the brain that declines with age (hippocampus). However, it was only dancing that led to noticeable behavioral changes – in the form of improved balance (Rehfeld et al., 2017).
Another study done by Scientists at Baylor College of Medicine and the University of Texas Health Science Center at Houston found that we may also underestimate the protective effects of a healthy gut microbiome.
Through slowing down the progression of tumors and the accumulation of amyloid-beta – a compound associated with Alzheimer’s disease – and increasing the production of the polysaccharide colanic acid, the promotion of a healthy gut bacteria associated was shown to have also protective effects against aging (Han et al., 2017).
What studies like this tell us is that there are a variety of things that we may not fully understand about aging, and likely, many helpful interventions.
But where do we start? What is considered normal as we age, and what requires medical or psychological evaluation?
There are actually clear and identifiable physical and psychosocial changes that affect people as they age, and can be addressed by a clinician who is educated and experienced with the aging population. Moreover, there are evidence-based strategies clinicians can use to assist older persons in the clinic or office as well as specific techniques to communicate effectively when sensory deficits are present in older adults.
Lastly, ensuring the highest level of functioning as clients age depends on a clinician who fully understands, and can help the client navigate, the continuum of care. All of this begins with a clinician who understands the challenges of aging – and perhaps more importantly, how to address and overcome them.

Communication with Elders is a 2-hour online continuing education (CE/CEU) course that offers strategies for improving communication with elders with the goal of improving treatment outcomes. With the increasing number of older people in the United States, it is vital for healthcare professionals to communicate effectively and respectfully with elders. Effective, appropriate communication with elders is important for many reasons. For psychotherapists and other mental health professionals, communication is the foundation of service delivery. The growth in the number and proportion of older adults is unprecedented in the history of the United States. Two factors – longer life spans and aging baby boomers – will combine to double the population of Americans aged 65 years or older during the next 25 years to about 72 million. By 2030, older adults will account for roughly 20% of the U.S. population (CDC, 2013).
The rapid aging of the U.S. population is being driven by two realities: Americans are living longer and there are proportionately more older adults than in previous generations due to the post-World War II baby boom. Many Americans are now living into their eighties and beyond. The leading edge of the baby boomers reached age 65 in 2011, launching an unparalleled phenomenon in the United States. Since January 1, 2011, and each and every day for the next 20 years, about 10,000 Americans will celebrate their 65th birthdays. In 2030, when the last baby boomer turns 65, the demographic landscape of our nation will have changed significantly.
The rising tide of an aging population has wide-ranging implications for almost every facet of American society. However, the most profound effects will be on our nation’s public health, social services, and health care systems.
The Committee on the Future Health Care Workforce for Older Americans, part of the Institute of Medicine, notes that the volume of older people may soon outnumber the supply of healthcare professionals trained in geriatrics. This is not the only problem, however. Older people offer challenges due to medical syndromes common to aging such as falls and malnutrition, which can lead to acute medical problems.
